Your IMMEDIATE attention is needed for the attached documents!!
These completed forms and payment must be received by the Treasurer no later than September 6th.
The first attached document is the latest P.O.D.
The second attached document is the ‘Registration, Admission and Transportation’ (RAT) form which has been used at ALL USS Grant reunions. This document provides the attendee numbers we require to arrange food and beverages in the Grant Hospitality Room (GHR), as well as the transportation from the hotel to off-site activities and to/from the cruise ship. Instructions for completing the RAT form are included with the form. The most recent POD is also included.
We CANNOT make changes in Jacksonville! The numbers and receipts from the RAT form are what we use to contract transportation and food for the scheduled reunion activities. ALL THE GRANT GROUP ACTIVITIES LISTED ON THE RAT FORM REQUIRE US TO MAKE A PRE-PAYMENT NO LATER THAN 9/16 TO GUARANTEE OUR NEGOTIATED REDUCED PRICE.
The third and fourth attached documents are a security form required for access to the submarine base and instructions for completing it. Military retirees/dependents with a current DoD I.D. (the new card without SSAN) do NOT need to complete this form. ALL other persons who plan to attend the activities at the Kings Bay Submarine Base (KBSB) MUST complete this form in blue or black ink, while using the specific instructions that are included with the form. Entry onto the Submarine Base will be denied to persons who do NOT possess a DoD ID card or have NOT completed this SECNAV 5512 security form.
THE SECNAV5512 FORM, THE RAT FORM AND THE REUNION PAYMENT MUST BE RECEIVED BY THE TREASURER NO LATER THAN September 6th.
